Sometimes the issue starts with the implementation approach itself. The project is built only so that something is up for now, without proper architecture, without attention to future development, without a clean structure, and without treating maintenance seriously. The result is usually a product that may seem to work at first, but with the first change, new development, or increase in users, its problems start to surface.
